DENTAL SURGERYPROCEDURES

Dental surgery includes a wide range of procedures that are related to the oral area. These include not only tooth removal (extractions), but also implantations, bone reconstructions using biomaterial, sinus lifts or raising the bottom of the maxillary sinus, as well as removal of lesions and cysts in the oral cavity, frenulum undercutting (both for children and adults) and procedures related to the surgical treatment of periodontal disease, i.e. periosurgery.

IMPLANTATIONS

The implantation procedure is preceded by a consultation with an implantologist. Computed tomography is performed during the visit. This is an examination thanks to which the doctor is able to determine the height of the appendix in the area of the planned implant, its width, as well as the course of the nerve and the distance from the maxillary sinuses.
When examining the inside of the mouth, the dentist also evaluates soft tissues such as mucosa, periodontium, and overall oral hygiene. Once the patient is diagnosed in terms of his health, he is able to proceed with the implantation.
The entire procedure is performed under local anesthesia. The implantologist makes a small incision in the gum, which exposes the bone. Then he drills a hole in it with special calibrated drills. An implant is inserted into the resulting hole with enough force to ensure stabilization.
The entire procedure takes about 20 minutes. The implant integration time takes from 3 to 6 months. After this time, we meet with the doctor to unveil the implant. Under local anesthesia, the doctor cuts the gum in the place of the previously integrated implant and replaces the closing screw with a healing and modeling screw. Its purpose is to shape the gum around the implant, which takes about 2 weeks. After that, we proceed with the prosthetic part.

SURGICAL REMOVAL OF EIGHTS

Eights are otherwise known as wisdom teeth. Their growth causes pain and discomfort, especially if they grow in the wrong position. Improperly growing wisdom teeth cause the risk of inflammation and damage to the structure of the adjacent teeth by exerting pressure on their roots.
Removal of wisdom teeth is carried out by a surgeon or, depending on the position of the tooth, by a maxillofacial surgeon. After taking an X-ray and evaluating the tooth, the doctor performs local anesthesia on the patient. The dentist incises the gum and removes the piece of bone that covers the wisdom tooth. Then he separates the roots of the tooth using appropriate drills. When the roots are loosened the tooth is removed with forceps.
The procedure ends with placement of a few stitches. The surgeon recommends a check-up after 7 – 10 days in order to remove the stitches and assess the healing of the site after the procedure. In addition, the patient is given an antibiotic and analgesic and anti-inflammatory drugs to alleviate inflammation, improve healing and reduce pain.

TEETH REMOVAL

Tooth extraction performed by a surgeon may result from the poor condition of the tooth and periodontium, as well as malocclusion.
An indication for tooth extraction may be advanced infection – when extensive decay reaches the pulp and can cause inflammation leading to purulent exudate.
The tooth may also be diagnosed for removal by an orthodontist due to overlapping teeth, making it impossible to put on braces.
Tooth extraction is performed under local anesthesia. The surgeon, proceeding to remove the tooth, first violates the ligament that surrounds the tooth and then, with the help of appropriate forceps, leads to loosening until finally the tooth is removed from the alveolar sack.
The post-extraction site is cleaned and the patient bites a cotton swab to stop the oozing blood. As a result, bleeding stops after 20 minutes and the placement of stitches is unnecessary.
Recommendations after tooth extraction include cooling the site after the procedure with cold compresses, eating soft and cold meals and also cool drinks.

SINUSLIFT / RAISING THE BOTTOM OF MAXILLARY SINUS / BONE REGENERATION

Sinus Lift is a procedure that increases the amount of bone tissue. This allows implants to be inserted in areas where bone atrophy is most common. These are the posterior segments of the jaw. Performing a sinus lift involves placing bone material over the bone defect.
As a result, it allows for bone growth, and after about 6 months, i.e. the time of integration of the bone graft material, the introduction of implants of the appropriate length.
Bone regeneration as well as the sinus lift procedure is designed to rebuild bone tissue where there are deficiencies. A special membrane is also used when the surgeon inserts the bone material. This is a membrane that is designed to protect the inserted bone graft material from soft tissue ingrowth. After a period of 3 – 6 months, the site is ready to begin implant-prosthetic treatment.

REMOVAL OF LESIONS AND CYSTS

The surgeon, after conducting a medical interview and taking the necessary photos before the procedure, determines the time and changes that occur. The procedure itself is performed under local anesthesia.
It involves the removal of lesions from the maxilla, mandible or changes in the oral mucosa. Depending on the size of the cyst, its removal is often combined with tooth extraction or resection of the roots of adjacent teeth. During the procedure, the cyst is enucleated together with the epithelium, and bone defects that may occur during its removal are supplemented with bone substitute material or, if possible, material obtained from the patient.
After each procedure, the lesion is sent for histopathological examination to determine the nature of the lesion and also to make an appropriate diagnosis.

FRENULUM UNDERCUTTING

Frenulum undercutting, both in children and adults, is completely painless. After administering local anesthesia, the doctor gently incises the membrane located in the specified area:

  • Under the tongue (between the lower surface of the tongue and the bottom of the mouth),
  • Above the upper lip (between the inner side of the lip and the gums between the ones),
  • Under the lower lip (between the inner side of the lip and the jaw gingiva),
  • by the cheek.

A scalpel, coagulation knife or laser can be used for the procedure. In all methods used, the bleeding is small, the mucosa heals up to 72 hours and there is no trace of the scar.

PERIOSURGERY

HOW IS A CYST FORMED?

The cause of cyst formation is long-term developing inflammation in the area of the tooth tissues. It may appear as a result of e.g. caries, lack of systematic hygienization visits as well as due to chronic diseases of the nose and throat.

WHAT ARE THE INDICATIONS FOR CHILD’S FRENULUM UNDERCUTTNG?

It all depends on the age of the child. In newborns, difficulty in sucking can be observed, while in older children one can notice difficulty in pronouncing sounds that require touching the tongue to the palate, lisping and even the appearance of a diastema (gap) between the first two teeth.
First of all, you should go to your primary care physician or speech therapist who will qualify your child to see a surgeon who performs frenulum undercutting.

WHEN IS THE BEST TIME TO REMOVE AN EIGHT?

It is recommended to remove wisdom teeth assessed by the dentist as improperly developed, when they press on the roots of adjacent teeth, due to orthodontic indications or when they’re damaged by caries.
The best age is from 16 to 20 years old when the period of bone development has come to an end. In addition, wisdom teeth are recommended to be removed in the autumn and winter, when lower temperatures aids to better healing of the post-extraction site.

ARE THERE AGE RESTRICTIONS FOR IMPLANTATION?

Patients age does not affect the possibility of placing a dental implant. The most important thing is the patient's health condition, after a medical interview with the implantologist, it is he who decides on the possibility of the procedure.

HOW TO TAKE CARE OF IMPLANTS?

Patient should take care of implants the same as of the rest of his own teeth. Proper oral and gum hygiene should be maintained. For this purpose, the use of special dental irrigators and dental floss is recommended. Additionally, it is recommended to purchase a special toothbrush with soft bristles as well as use mouthwashes and toothpaste with an antibacterial substance - chlorhexidine.

WHAT ARE THE INDICATIONS FOR A BONE AUGMENTATION PROCEDURE?

Bone augmentation precedes the dental implant procedure when the quantity and quality of the patient's bone is insufficient. In addition, it is recommended by the surgeon when there appears advanced periodontal disease, resulting in significant bone loss, in case of patients after extractions where bone loss has occurred during healing, or with periapical lesions.

WHAT DOES DENTAL SURGERY DO?

Treatments in the field of dental surgery are those that deal with the surgical treatment of the oral cavity, as well as parts that adhere to it - such as the maxilla or mandible.
Dental surgery is not only the extraction of deciduous, permanent and retained teeth, but also alveolar plastics, apical resections, cysts enucleation, incision of odontogenic abscess and bone reconstruction procedures.

HOW DOES QUALIFICATION FOR SELECTED TREATMENTS LOOK LIKE?

Usually the indication for dental surgery is the removal of retained teeth, but not only. These also include tooth tissue disease, extractions due to orthodontics, broken or loosened teeth. We also come to the surgeon because of the root apex resection, e.g. caused by tool fracture during root canal treatment or a periapical cyst.
The success of implant treatment is based on adequate quantity and quality of bone. When atrophy occurs, bone reconstruction - otherwise known as augmentation - is performed right by a dental surgeon. This procedure not only allows for successful implant placement, but also protects against the negative effects of bone atrophy. We also go to the surgeon when the structure of the frenulum, especially in child's case, is abnormal and causes problems not only in hygiene, but also in pronunciation and the sucking reflex. The procedure is short and is performed under local anesthesia or when the situation requires it, with help of inhalation sedation.

WHAT DOES THE PREPARATION FOR DENTAL SURGERY LOOK LIKE?

Preparation for dental surgery procedures begins with an oral diagnosis. The most important is to take an X-ray or computed tomography. Based on these, doctor is able to determine the problem, plan the entire procedure and together with patient’s caregiver create a treatment plan that includes all the steps of each procedure.
The patient is informed about the entire treatment process and is provided with all post-treatment recommendations that he should follow the next few days. Any surgical or dental procedure is an interference with the body. Therefore, it is extremely important to maintain sterile treatment conditions. In the office, all standards in this area are observed, and patient is cared for at the highest level, thanks to which he feels safe. WHAT ARE THE RECOMMENDATIONS AFTER DENTAL SURGERY PROCEDURES?

After dental surgery, the most important thing is the patient's recuperation, so that the body can recover. It is important to avoid physical activity. After the procedure, painkillers and an antibiotic (if prescribed) should be used according to the doctor's instructions.
The use of cold compresses will minimize swelling. For the next few days, it is worth using a soft and semi-liquid diet - avoid hard, hot and spicy foods so as not to irritate the post-treatment site. Avoid smoking and drinking alcohol, don’t sleep on the post-treatment site, don’t touch it.
