TREATMENT OF PERIODONTITIS

Treating periodontitis can be a time-consuming process. During the visit, the dentist identifies the type of gum inflammation and individually selects the most appropriate and effective treatment methods. Antibiotic therapy may sometimes be necessary – dentist takes samples for laboratory tests to determine which groups of bacteria have attacked the patient’s gums and caused the inflammation.
Subsequently, the appropriate antibiotic can be prescribed. The methods and duration of treatment depend on the severity of the disease. Tartar and bacterial plaque must be removed not only from the teeth but also from the pockets – the depth of these pockets is assessed by the dentist as it often determines the next steps, which may even involve surgical treatment.
In some cases, it is necessary to regenerate periodontal tissues, such as bone and gums. Fortunately, in most cases, the disease can be cured by a dentist. An essential part of the treatment is oral hygiene, which includes scaling, sandblasting, and fluoridation. Maintaining a proper diet (reducing sugar intake), providing the body with vitamins and minerals, and taking care of overall immunity are also crucial.

CURETTAGE

Curettage is a medical procedure in the field of periodontology, which is a branch of dentistry that deals with the treatment of periodontal diseases – tissues surrounding the tooth.
There are two types of curettage:
Open curettage – flap procedure.
This procedure involves cleaning the roots in cases of very deep pockets. Unlike closed curettage, it is performed after making an incision in the gum and exposing the root surfaces. After the cleaning is completed, the gums are sutured. The curettage procedure is usually carried out in cases of pathological enlargement of gingival pockets. Under normal physiological conditions, these pockets should not be larger than 2 mm. If their space deepens excessively, they start to accumulate food particles, exfoliating epithelium, and microorganisms residing in the oral cavity. This can lead to the development of inflammation and numerous associated complications.
Indications for performing curettage include: advanced gingival inflammations, periodontal diseases, including periodontitis which leads to tooth loss.

Gingival recession refers to exposed tooth root surfaces, which not only look aesthetically unpleasant but can also cause increased sensitivity, elongation of the tooth surface, and an increased risk of tooth decay.
The most common cause of exposed tooth roots is improper brushing technique (using horizontal strokes with excessive force) or inadequate oral hygiene, leading to the development of gum inflammation or bite abnormalities causing tooth overload.

GUM PLASTIC SURGERY

The main indication for gum plastic surgery is excessive gum tissue that covers the teeth.
This condition is known as “gummy smile,” and it occurs when more than 2 mm of gums are visible when smiling, making the tooth crowns appear shorter.
Another indication is gum defects or recession, where the tooth root is excessively exposed. Gum recession can lead to increased sensitivity to high or low temperatures, sour and sweet tastes, and sometimes even toothpaste with an intensely minty flavor.

Frenulum undercutting is a short and painless procedure aimed at increasing the mobility of the upper lip. If the frenulum is too short, it pulls and tugs on the gums during facial movements, causing exposure of the necks and roots of the upper incisors. It can also result in a cosmetic defect such as diastema (a gap between the incisors).

Gingivectomy is a procedure aimed at removing an excessive amount of gum tissue covering the teeth. Overgrown gums can hinder proper oral hygiene. It may also lead to inflammatory conditions such as periodontal pocket inflammation or aesthetic defects known as a “gummy smile.”
During the procedure, a specialist lengthens the tooth crowns and eliminates unnecessary, excessively grown gum tissue. This procedure can be performed using a diode laser, making it painless and highly precise.

WHAT IS PERIODONTITIS?

It is an infection that attacks the tissues surrounding the teeth, leading to inflammation. The disease is caused by bacteria that multiply in the oral cavity due to improper oral hygiene or incorrect tooth brushing.
Periodontitis is a condition that most commonly occurs in people over the age of 40, but it can also appear much earlier. Periodontal tissue infection causes inflammation, which causes our teeth to start moving. At first, the symptoms of the disease are unnoticeable, but as periodontitis progresses, they become increasingly severe.

CAN PERIODONTITIS BE CURED USING HOME REMEDIES?

No home remedies will help prevent or cure periodontitis. Only conscious and diligent oral hygiene can reduce the risk of developing this disease.
In addition to thorough tooth brushing, it is important to regularly visit a dentist to remove plaque and tartar. In later stages of the disease, specialized cleaning of gum pockets by a periodontist is required.

WHAT IS PERIODONTITIS AND WHO IS MOST SUSCEPTIBLE TO IT?

Periodontitis is a disease characterized by inflammation of the periodontium. Its advanced stage can lead to tooth mobility and even tooth loss.
The main culprit of periodontitis is improper oral hygiene. Every time we consume food or beverages, residue remains on our teeth. If we don't remove them in time, bacteria and fungi will accumulate on our dental plaque (especially at the junction of teeth and gums). It is mainly these microorganisms and the resulting plaque, which under the influence of saliva, hardens into tartar, that contribute to development of periodontal diseases, including periodontitis.
Additional factors that contribute to the progression of the disease include being over 35 years old, smoking cigarettes, excessive alcohol consumption, coexisting conditions (such as diabetes, cardiovascular disease, compromised immune system), and hormonal fluctuations (especially during menopause, pregnancy, or adolescence).

WHAT ARE THE CAUSES OF PERIODONTITIS? IS IT AN INHERITED DISEASE?

The causes of periodontitis can vary, but it generally occurs due to a weakened immune system and improper oral hygiene. A weakened immune system is less capable of defending against bacteria, including those in the oral cavity. When a weakened immune system combines with poor oral hygiene, bacteria can multiply more easily and cause inflammation in the tooth socket.
The causes of immune system weakening can be both physical and psychological, which means that determining the exact cause of periodontal diseases is often impossible. However, it can generally be said that good oral hygiene can prevent many cases of periodontal disease.

WHAT DOES PERIODONTITIS TREATMENT CONSIST OF?

First, periodontitis is assessed using a probe to measure the depth of the periodontal pockets, which quickly indicates to the dentist whether there is suspicion of gum inflammation. Additionally, a blood test can be conducted to determine if there is a more pronounced inflammatory condition in the body, which helps the dentist confirm the diagnosis.
Once periodontitis is diagnosed, treatment can begin. During the procedure, dentist first aims to thoroughly clean the teeth and periodontal pockets to eliminate bacteria. However, if the bacteria are located too deeply or near the root, a minor surgical intervention may be necessary. In such cases, dentist removes a part of the gum to improve visibility and create a larger working space around the tooth, and then removes all the bacteria.

WHAT IMPACT DOES PERIODONTITIS HAVE ON IMPLANT TREATMENT?

Periodontitis does not exclude implant treatment. However, it must be preceded by the removal of inflammatory conditions by a periodontal specialist.

WHAT IS THE PREVENTION AND RECOMMENDATIONS FOR PEOPLE PRONE TO PERIODONTITIS OR AFTER TREATMENT?

Using dental floss to thoroughly clean the interdental spaces. Using moderate force when brushing teeth (without pressure, using circular motions). Regularly brushing the tongue, preferably with a special scraper. For tobacco smokers, maintaining optimal oral hygiene and having regular dental check-ups. Additionally smokers have a 5 times higher risk of periodontitis compared to non-smokers.

WHAT IS GINGIVAL RECESSION?

Recession is a receding of the gum and bone from the tooth. This firstly results in an aesthetic defect, especially when it affects the front teeth. Secondly, the exposed root, which was previously protected by the gum and bone from the unfavorable oral environment, is directly exposed to bacterial attack and irritating factors from the oral cavity.

WHAT ARE THE CAUSES OF GINGIVAL RECESSION?

-Incorrect toothbrushing technique (horizontal scrubbing motions with excessive force or using a toothbrush that is too hard).
-Chronic low-grade gingivitis caused by improper oral hygiene and prolonged inflammation around the tooth neck due to the presence of bacterial plaque.
-Improper attachment of frenulum (lip, tongue, and cheek).
-Orthodontic treatment involving expansion of the dental arches.
-Improperly placed fillings or dental crowns.
-Teeth grinding (bruxism) or clenching.

WHAT ARE THE FIRST SYMPTOMS OF GINGIVAL RECESSION?

Pain while eating, bleeding, gum lowering, exposure of the tooth neck and root - these symptoms often indicate gingival recession.

WHAT ARE THE METHODS AND PROCESS OF TREATING GINGIVAL RECESSION?

A dentist specializing in periodontal diseases - periodontist, is responsible for treating gingival recession. Depending on the severity and underlying cause of the recession, he may use different treatment methods.
Covering the gingival recession with composite material - it helps reducing sensitivity and covers the exposed part of the tooth.
Tooth root surface smoothing and removal of calculus - it is done above and below the gum line to create a smooth and unitary area where the gums meet the teeth. The dentist may also recommend antiseptic mouth rinses. Multiple treatments may be necessary to achieve the desired results, and if patient experiences excessive pain or hypersensitivity, local anesthesia can be used.
Surgical intervention - involves rebuilding the receding gum tissue.
Gum grafting - it is the most radical method described. It is used in advanced stages of the disease where a significant part of the gums has been lost. Gum grafting involves covering the exposed tooth root by transplanting tissue from the palate or another area of the oral cavity to the recessed area.

WHAT ARE THE CONSEQUENCES OF NOT TREATING GINGIVAL RECESSION?

Untreated gingival recession can lead to various periodontal diseases, and in more severe cases, gingival atrophy can occur. Although dentistry is a highly developed field of medicine nowadays and can treat even advanced diseases, it is not worth risking unnecessary dental problems. Untreated gingival recession can also result in inflammatory conditions affecting the bone, which can impact overall health.

PREVENTION - HOW TO PREVENT GINGIVAL RECESSION?

To prevent the condition or its recurrence in the future, it is important to maintain proper oral hygiene and regularly visit the dentist for check-ups. Brush your teeth following the dentist's instructions. Use for this an appropriate, soft-bristled toothbrush. Regularly remove dental calculus at the dental office and promptly treat any cavities. These preventive measures will positively impact the condition of your teeth and help avoid gingival recession.
